The Vermont Parent Survey - Enter to WIN $100!

The Vermont Parent Survey on Youth Substance Use is closing soon! If you are a parent of a student in grades 6-12, please take the survey! Seven lucky Vermont parents have already won a weekly random drawing this fall for $100. You could be next or even win the $500 grand prize! This short survey of parents' thoughts on teen alcohol and drug use is being conducted by Pacific Institute for Research and Evaluation in partnership with the Vermont Department of Health and is designed for parents with children in grades 6-12 in Vermont. Respondents will be eligible to enter a drawing for a weekly cash prize of $100 and a grand prize of $500 at the end of the survey. The survey itself collects no personally identifying information and survey responses cannot be linked with contact information entered on the prize drawing site.
